Eligibility Criteria
Must be an Australian citizen, permanent resident, or Protected Special Category Visa holder.
Age between 7 and 65 when applying.
Have a permanent disability affecting daily life.
Need assistance or equipment for daily activities.
How to get an NDIS Plan?
Phone Request: Call 1800 800 110 for a verbal access request.
Digital Submission: Download the ARF, complete it, and email to NDIA.
Mail-In: Print ARF, complete and mail to NDIA.
Local Office: Visit or contact your local NDIA office for a mailed ARF.
Getting Help for your NDIS Application
NDIA staff and Partners in the Community can assist in the application process.
Early childhood partners and local coordinators can help gather evidence for your application.
If you've started an application, you'll be referred to a local NDIS partner for review and guidance.
Contact the National Contact Centre at 1800 800 110 for a referral to an NDIS partner.
